What is a nasal septum perforation?
Nasal septum perforation, or perforation of the nasal septum, is the appearance of an abnormal opening that connects both nasal cavities, causing bothersome symptoms such as nasal obstruction, mucus, nasal crusts and frequent nosebleeds.
Causes
The causes that can lead to a perforated nasal septum are usually related to previous nasal surgeries. However, it is also common to find cases of perforated nasal septum due to drugs or trauma. Other, less frequent, causes of nasal septum perforation include systemic diseases that cause inflammation of the nose and sinuses, which ultimately affect the blood circulation of the nasal septum, leading to tissue loss.
Symptoms of nasal septum perforation
Nasal septum perforations are usually asymptomatic, but can sometimes cause a significant deterioration in quality of life.

Diagnosis
For the diagnosis of a nasal septum perforation, a nasal endoscopy or scan must be performed. This is an in-consultation procedure that will allow visualisation of the perforation site, assessment of the state of the edges and the osteocartilaginous support, removal of crusts, and measurement of diameters, which will be useful for planning the intervention.
Treatment
Treatment for a perforated nasal septum should be carried out whenever the patient presents with symptoms.
There are perforations that do not produce symptoms, and in these cases, no treatment is necessary. However, in the case of a nasal septum perforation that produces symptoms, the best alternative is the reconstruction of the perforated nasal septum through surgery.
